1) Applicant’s Driver’s License
2) Current Vehicle Registration*
*If the vehicle registration is not in the applicant’s name or in the name of an applicant’s parent with the same last name, a notarized Vehicle Use Affidavit(PDF, 102KB) from the vehicle’s owner must be provided authorizing the use of the vehicle and stating the relationship of owner to applicant. Vehicle owner must be a family member (mother/stepmother, father/stepfather, son, daughter, sister, brother, grandparent, or grandchild). If the vehicle is corporately owned, please use Corporate Vehicle Affidavit(PDF, 136KB).
3) Lease*
Fully executed lease or sublease with all pages and signatures.
*If the lease does not have the full street address and apartment number (if any), a move-in email or a signed statement from your landlord giving this information is required.
4) Additional Proof of Residency
One of the following which must be in the applicant’s name and show the lease address:
- Utility, phone, cable or internet bill
- Renter’s insurance certificate
- Bank statement
- Official mail from an educational or medical institution
- Voter’s registration card
- Applicant’s driver’s license with lease address
All proofs of residency must be in applicant's name, include the address listed on the lease, and be current or dated within the last 60 days. Applications that fail to include this qualifying information will be rejected.
Any person who submits an altered or otherwise false document will be denied a permit and may have their privilege to reapply for a permit revoked.
